  5. Internet Usage Policy and Agreement
  6. Birth Certificate
  7. Immunization records (or signed waiver available from school secretary)
  8. Proof of TB test (or signed waiver available from school secretary)
  9. New 2011-12:  7th & 8th grade TDAP Booster (or new signed waiver for TDAP available from school secretary)
  10. Proof of residence (copy of utility bill, etc.)
  11. ***First time kindergarten and 1st grade students require a Physical and Dental visit (or waiver). ***
