Enrollment Requirements

  1. Meet or talk with an Advising Teacher or Administrator to see if Mountain Home is right for your child.

  2. Complete the online Student Registration.

  3. *** First-time kindergarten and 1st-grade students require a Physical (or waiver) and Dental visit (or waiver). Click here for forms. View our Early K Admission policy below.

  4. Submit a copy of the Birth Certificate.

  5. Submit Immunization records or signed waiver (only Medical exemptions are accepted as of 7/1/16.). Contact us if you have further questions. View our Student Health and Immunization policy below.

  6. Submit proof of TB test (or signed waiver, which requires a health care provider's signature).

  7. 7th grade TDAP Booster or TDAP exemption signed (only Medical exemptions are accepted as of 7/1/16).

  8. Submit proof of residence (copy of utility bill, etc.).
